Photocopy of a declassified questionnaire used to determine if the person named is to be considered an enemy alien. This page covers questions 109a - 111 of 111. On this last page of the questionnaire, Koyama states that he has never belonged to an organization whose purposes has been the overthrow of the United States government, nor has he engaged in distributing any foreign propaganda. He says that the purpose for coming to the United States was to study dentistry, to become a permanent resident, and to establish a happy home.
