Photocopy of a declassified questionnaire used to determine if the person named is to be considered an enemy alien. This page covers questions 66a - 74 of 111. On questions concerning U.S. citizenship, Dr. Koyama states that he is not currently a citizen due to current United States laws barring him from doing so. He has resided in the United States for 28 years and has not filed a petition for naturalization. He lists that he has remarried and that his former wife, Miho Fukuda, is deceased.
