The Notary Public for the form was for Hunt, Idaho, but was crossed out to reference Portland, Oregon. Mrs. Goodenough writes about a Japanese organization that Dr. Koyama belong to, though she cannot remember the name. She adds that that Dr. Koyama often discussed the group's business at her house and said that the goal of the group was to raise money for Japanese widows and orphans and to help with resettlement. She ends the document stating her support for Dr. Koyama and attesting to his loyalty to the United States.",
