NEXT, study the printed form called "APPLICATION FOR FINAL DISPOSITION OF REMAINS."
THEN read slowly the little gray booklet.
FINALLY, look over the other enclosures.

NOW, after you have thought it all over and decided where you want your loved one buried, you are ready to fill out the application form. If you want help in filling out the form, any one of the following in your community will be pleased to assist you:

YOUR LOCAL FUNERAL DIRECTOR.
YOUR RED CROSS CHAPTER.
YOUR LOCAL POST OF THE AMERICAN LEGION, VETERANS OF FOREIGN WARS, [ILLEGIBLE], OR OTHER VETERANS' ORGANIZATION.
THE NEAREST OFFICE OF THE VETERANS ADMINISTRATION OR OTHER VETERANS' AGENCY.

THIS help will be given to you free of charge.

BE sure you have a notary public certify your signature on page 2 of the form.

IMPORTANT

If there has been a change in the status as next of kin of the person to whom this letter has been addressed, the next person in line of kinship to the deceased should forward with the application documentary evidence to establish this fact, i.e.,

a. In the case where the next of kin has died a certified copy of death certificate, or

b. In the case where the decedent's widow has re-married a certified copy of marriage record issued by a clerk of an office of record.
