Every Japanese, German or Italian alien, or person of Japanese ancestry, resident in Military Area No. 1 who changes his place of habitual residence is required to obtain and execute a "Change of Residence Notice" in advance of such change in residence. (Proclamation No. 1, Commanding General, Western Defense Command, pursuant to Executive Order No. 9066.) Failure to comply with this regulation is subject to a heavy penalty.

Forms for this purpose are available at any United States Post Office within the States of Washington, Oregon, California and Arizona.

Cards are required for all members of a family.
