Photocopy of a declassified questionnaire used to determine if the person named is to be considered an enemy alien. This page covers questions 98b - 102b of 111. Koyama continues to answer "No" to all questions regarding his or family involvement in foreign governments or their military. He also states that he has never aided an alien in entering the United States. He then lists all the organizations and their addresses that has participated in within the last five years. He lists the Japanese M.E. Church, the Oregon Dental Association, the American Red Cross, the Oregon Japanese Golf Club, the National Geographic Society, and the Japanese Chamber of Commerce. He marks that none of these organizations have military motives or use fire arms.
