DESCRIPTION

Photocopy of a declassified questionnaire used to determine if the person named is to be considered an enemy alien. This page covers questions 11a - 15a of 111. Dr. Koyama states that he has a Japanese passport, but is unsure of the number as it is in a safety deposit box at the First National Bank of Portland. He did register under the Alien Registration Act of 1940 and that his number is 4043236. He does not have a United States Social Security number and has never been charged with a crime while abroad.
